Fingerprint Privacy Notice and Consent Workflow and Template

Use this sample document to help inform employees about how fingerprint authentication works with MedixSafe and obtain their consent before enrollment.

Optional consent workflow

If your organization requires a formal consent process before fingerprint enrollment:

1. Download the template and customize the employer name, retention period, and contact information to reflect your organization’s policies.
2. Have each user review and complete the consent form before enrolling their fingerprint. Retain completed forms according to your organization’s policies.
3. Once consent has been collected, an administrator can select a biometric authentication mode in the device settings and check the biometric consent acknowledgment box.
4. Enroll the users’ fingerprints.
